Request prescription Medicines

Established in 1966, our Affordable Prescription Program has empowered uninsured and underinsured Michigan residents, age 18 and older, to access vital prescription medicines without financial hardship.

Born from a faith-rooted commitment to compassion and service, we’ve spent nearly six decades lifting the burden of high drug costs so you can focus on staying healthy and caring for your loved ones.

All clients enrolled in the Affordable Prescription Program must complete an annual recertification. Click here to begin your recertification.

Complete our Prescription Request Form

Submit your request: Complete our Affordable Prescription Program application form telling us where the aid will go and who will receive it.
State-licensed pharmacy: Once approved, we fill your prescriptions safely and professionally.
Delivery or pick-up: Have your medicines mailed to your home (shipping fee applies) or stop by to pick them up.

Qualifications

You may be eligible if you:

Hold a valid Michigan driver’s license
Are 18 years of age or older
Earn $44,800 or less per year (single) or $51,200 or less per year (couple)
Do not have prescription drug coverage (even if you have health insurance)
Are not enrolled in Medicaid or have Medicaid but need help with medicines it doesn’t cover
